This enrollment process is for all NEW students enrolling in Transitional Kindergarten (TK) through grade 12. If your student was attending an MUSD school last year in grades TK-12, you do not need to complete this process.
Requirements to Register New Students
Students enrolling for Kindergarten must be five years of age by September 1, 2024 (EC 48000a). Students are eligible to enroll for Transitional Kindergarten (TK) if their birthday is between September 2, 2024 and June 30, 2025. Students whose birthday is between July 1, 2025 and September 1, 2025 will be waitlisted & enrolled in Transitional Kindergarten as space permits.
Please bring the following documentation to register:
1. Verification of Student Date of Birth:
✓ Birth Certificate, Passport, Certified Hospital Birth Record, or duly attested Baptismal Record.
2. Verification of Immunizations - Provide a copy of the immunization record at the time of enrollment. Your child must have all vaccinations needed. The following immunizations are required for school entry:
✓ 5- DTap, 4- Polio, 3- Hepatitis B, 2- MMR, 2- Varicella.
✓ Tdap is required for students enrolling in the 7th or 8th grade.
✓ Health Examination For School Entry Required.
3. Proof of Residency (ONE of the following is required) - The document must be dated within 30 days:
✓ Gas, Water, Electric, Sanitation, current lease/rental agreement or escrow documents. Bill must have the parent’s name and date on the document. If unable to provide proof of residence, ask for an Affidavit of residence and return with a proof of address of the person who signed the affidavit.
4. Parent/Guardian Driver's License or other Picture ID: Guardianship documents are needed if you are the guardian of the student being enrolled. If you do not have guardianship paperwork and are the caregiver for the student, you must complete a Caregiver's Authorization Affidavit. The Caregivers Authorization form is available upon request and needs to be renewed each school year that the student attends. A driver’s license or picture ID is not an acceptable form of residence verification.
5. Oral Health Assessment:
✓ Only for Transitional Kindergarten and Kindergarten
6. Legal Custody papers – if there are any legal court orders or foster placement papers.
7. Special Education:
✓ If your child has an IEP Individualized Education Program
